By signing and submitting this form, the undersigned declares, and certifies, the following as true and correct as of the date this form is signed:

I am a member of ACMP and the ACMP UK Chapter, so a member in good standing.
I am 21 years of age or older and am a resident of the United Kingdom.
I have been a member of ACMP for 12 consecutive months (1 year) or more by the time of the start of the term for which the Director is appointed or nominated. (If you do not meet this statement, cross out before signing).
I am not an employee of ACMP nor am I the spouse, dependent, or member of an ACMP employee’s immediate family.
I am not aware of any conflict of interest, or any potential conflict, between my personal interests and those of ACMP Global or ACMP UK Chapter.
I have no undischarged bankrupt or have not committed offences related to fraud.
